Birth Certificates

The following individuals are eligible to receive a copy of a birth certificate:

  1. Individual named on the certificate (18+ years old).
  2. Parent(s) of individual named on the birth certificate (if parent named on birth certificate).
  3. Grandparent(s) or Great-Grandparent(s) of the individual named on the birth certificate. (Copy of birth certificate of your child/grandchild to prove relationship).
  4. Sibling, brother or sister older than 18 year with same parent(s) of the individual named on the certificate. (If not born in Madison County, copy of birth certificate to prove relationship).
  5. Spouse of the individual named on the certificate (Marriage license to prove relationship)
  6. Adult child or Grandchild or Great-Grandchild, 18 years or older, of the individual named on the birth certificate. (Copy of own and/or your parent’s or grandparent’s birth certificate to prove relationship if not born in Madison County).
  7. Legal Guardian of the individual named on the birth certificate. Legal guardianship papers with raised court seal.
  8. Child Protective Services Representative, with agency ID and original court placement document(s) with raised court seal.  Note: MCHD cannot issue a birth certificate to foster parents.
  9. Court Order to release record.  Must show original document(s) with raised court seal.  Note: MCHD cannot issue a birth certificate based on a Power of Attorney document.
  10. Birth Certificate copy is $10.00
