COVID-19 VACCINATION

Covid-19 vaccine available at the

Madison County Health Department

for any Hoosier age 12 and older

12+ CAN ONLY GET PFIZER, SCHEDULE AT SITE LISTED AS 'PVAX'

  • You must bring proof of age: driver’s license, birth certificate, letter from doctor, etc.

  • No 4-month grace period – you MUST be eligible at the time of vaccination

  • Parental consent is required for minors.
