COVID-19 TESTING AND VACCINATION (AGES 5+)

VACCINE NOTES

All three vaccines, and boosters, are available.

For Minors:

  • 5+ CAN ONLY GET PFIZER, SCHEDULE AT SITE LISTED AS 'PVAX'

  • Parental consent is required for minors.

  • Bring proof of age: driver’s license, birth certificate, a letter from doctor, etc.

  • No 4-month grace period – you must be eligible at the time of vaccination

TESTING NOTES
 

For any symptomatic person.  Symptoms may include: Cough, Shortness of Breath or Difficulty Breathing, Fatigue, Muscle or Body Aches Headache, New Loss of Taste or Smell, Sore throat, Congestion or Runny Nose, Nausea or Vomiting, Diarrhea
