BIRTH CERTIFICATE
To obtain a copy(s) of a Birth Certificate...
walk-ins - Monday through Friday 8am to 4pm or by mail, simply download and complete and return the application (below), ALONG WITH:

A copy of your ID, We suggest one of the following: a copy of your drivers license, a copy of your signed social security card, a copy of your car registration)

WHO QUALIFIES
TO RECEIVE A
BIRTH CERTIFICATE?
-
Individual named on the Birth Certificate (18+ years old).
-
Parent(s) of person on the record (if parent named on record).
-
Grandparent(s) or Great-Grandparent(s) of the individual named on record. (Copy of birth certificate of your child/grandchild to prove relationship).
-
Sibling brother or sister, older than 18 years old with the same parent(s) of the person named on the record. (If not born in Madison County, copy of their birth certificate is required to prove relationship).
-
Spouse of person named on record (marriage license to prove relationship).
-
Adult Child or Grandchild or Great-Grandchild (18+ years old) of the person named on the record(copy of own and/or your parent's or grandparent's birth certificate to prove the relationship if not born in Madison County).

Legal Guardian of person named on the record. Legal guardianship papers with raised court seal.

Child Protective Services Representative with agency ID and original court placement document(s) with raised court seal. Note: MCHD cannot issue a birth record to foster parent(s).

Court Order to release record. Must show original document(s) with raised court seal. Note: MCHD cannot issue a birth record based on a Power of Attorney (POA) document.

GENEALOGY SEARCHES
CONDUCT A SEARCH
Our Vital Records Division can conduct genealogy searches!
Begin by downloading and completing the information that you know (we will fill in the blanks). Return to the Madison County Health Department at:

Requests will be limited to five per customer with a SEARCH FEE OF $5.00 dollars per visit, regardless if a record is found.

Fun Fact:
Though recording was not required until 1907, some of our records begin in 1882!
