Vital Records

1. Individual named on the Birth Certificate (18+ years old).
   
   
2. Parent(s)of person on the record (if parent named on record).
   
   
3. Grandparent(s) or Great-Grandparent(s) of the individual named on record. (Copy of birth certificate of your child/grandchild to prove relationship).
   
   
4. Sibling brother or sister, older than 18 years old with the same parent(s) of the person named on the record. (If not born in Madison County, copy of their birth certificate is required to prove relationship).
   
   
5. Spouse of person named on record (marriage license to prove relationship).
   
   
6. Adult Child or Grandchild or Great-Grandchild (18+ years old) of the person named on the record(copy of own and/or your parent's or grandparent's birth certificate to prove the relationship if not born in Madison County).
   
   
7. Legal Guardian of person named on the record. Legal guardianship papers with raised court seal. 
   
   
8. Child Protective Services Representative with agency ID and original court placement document(s) with raised court seal. Note: MCHD cannot issue a birth record to foster parent(s).
   
   
9. Court Order to release record. Must show original document(s) with raised court seal. Note: MCHD cannot issue a birth record based on a Power of Attorney (POA) document

Birth Certificates are $15.00 per copy.

To obtain a copy(s) of a Death Certificate:
walk-ins – Monday through Friday 8am to 4pm, a mail-in request by downloading and completing an application , or online orders by clicking the link Vital Director below.

Online Order request:
Online orders will be processed by the Madison County Health Department through VitalDirector by Permitium. VitalDirector is the only Madison County Health Department authorized and approved vendor for ordering vital records online.

Mail-in request:
Print the mail-in application, complete the required information, include a self-addressed stamped envelope, $15 per copy. Once we receive your application through the mail, we will process it and put back in the mail within 2-4 business days.

Death certificates cost $15.00 a copy.

What is needed to complete a Paternity Affidavit or a Paternity Affidavit Upon Marriage:

• The father must be the biological father of child
• The brochure “Establishing Paternity” given prior to making an appointment
• After reading brochure, call and make an appointment
• Must have I. D. (Driver’s license, Passport, State I. D.)
• If Paternity Affidavit Upon Marriage must have Marriage License
• Cost $25.00 to complete a Paternity Affidavit
• If you wish to purchase a birth certificate the cost is $15.00 per copy. More info here.
